You consent to receive at-home screening test kits, including but not limited to FIT (Fecal Immunochemical Test) kits and/or HbA1c test kits, provided by SNT Biotech. Meridian Medicaid Plan (Meridian) had partnered with SNT Biotech to provide you with this screening service.
You understand that these test kits are intended for personal use only and are not a substitute for professional medical advice, diagnosis, or treatment.
You agree to follow the instructions provided with the test kits accurately and to use the kits only for their intended purpose.

You consent to receive communications from SNT Biotech and its affiliates, including notifications, reminders, and follow-ups related to the at-home screening test kits.
Communications may be sent via email, SMS/text message, phone calls, or mail. You understand that message and data rates may apply.
You may opt out of these communications at any time by following the unsubscribe instructions provided in the communications or by contacting us directly at 1-855-792-8378.

You consent to the collection, use, and sharing of your personal information, including health-related information, as described in SNT Biotech's Privacy Policy.
Your personal information will be used solely for purposes related to the athome screening program, including processing your test results, providing customer support, and improving our services.
SNT Biotech will take reasonable steps to protect your personal information from unauthorized access, use, or disclosure.
You consent to SNT Biotech sharing your test results and related metrics with your health plan and primary care physician (PCP) for the purpose of closing care gaps and improving healthcare outcomes.

SNT Biotech and its affiliates shall not be liable for any direct, indirect, incidental, consequential, or special damages arising out of or in connection with the use of the at-home screening test kits, including inaccuracies or errors in the test results, misuse of the test kits, or any health-related issues arising from the use of the test kits.
You agree to indemnify and hold harmless SNT Biotech and its affiliates from any claims, damages, or expenses arising out of your use of the test kits or any breach of these terms and conditions.

These terms and conditions shall be governed by and construed in accordance with the laws of the jurisdiction in which SNT Biotech is based, without regard to its conflict of law principles.
